In a landmark accomplishment, a surgical team led by Dr Vikas Bhise, designation consultant urologist at Medicover Hospital, Navi Mumbai, successfully performed an intricate emergency surgery on a 55-year-old man with autosomal dominant polycystic kidney disease (ADPKD). The patient arrived at the emergency department in critical condition, experiencing severe abdominal pain and blood in the urine. The patient underwent a nephrectomy to remove a massive 35 cm kidney mass. The patient has been discharged from the hospital, and is now comfortably back to his daily routine, living a healthier and more active life.
Patient Mahesh Yadav (name changed)* was on dialysis for several years due to autosomal dominant polycystic kidney disease (ADPKD), a genetic disorder characterized by cyst formation in the kidneys. He was under treatment for the same under Dr Amit Nagrik senior nephrologist at Medicover Hospital and planned for a kidney transplant. He was relying heavily on his family for support with even the simplest daily tasks. Each day was a struggle, filled with pain, uncertainty, and the emotional burden of watching his loved ones shoulder his care.
Dr Vikas Bhise, Urologist, Medicover Hospital said, “On arrival in an emergency patient lost a lot of blood from urine and was in hypotension. He had a known case of ADPKD for which he was following up. In ADPKD, the kidney is replaced by a fluid-filled cyst, and it's a rare genetic disorder and rarer to present with such a huge mass. Over time, his abdomen became severely distended, and he experienced worsening discomfort. When the bleeding started, he knew his condition had become life-threatening. This was a sporadic and challenging case due to the size and complexity of the kidney mass which was 35 cm and weighed. The kidney was not only enlarged but the whole abdomen was tensed, making surgical removal meticulous and demanding. We opted for an open approach because cysts occupied the entire abdomen.”
Dr Bhise added, “The case was challenging as normal anatomy of the abdomen is lost. Thanks to a dedicated surgical team and careful planning, the surgery was completed in two hours with minimal blood loss, and the patient required no postoperative transfusion. Not treating him at the right time could have led to complications like shock and severe blood loss. Seeing him recover so well is immensely gratifying. Discharged just three days after surgery and mobilized the very next day, he is now on the path to regaining a better quality of life.”
